Who needs to update their data in PESEL UKR?

Updating is mandatory for you if:

- You arrived in Poland after February 24, 2022 and received PESEL UKR.

- Your foreign passport has expired.

- You have received a new passport or other identity document.

- You have changed your surname, name or other personal data (e.g. due to marriage or divorce).

1. A valid foreign passport or a new passport.

2. Documents on change of personal data (if there were any changes): marriage/divorce certificate, court decision, certificate of change of name, etc.

3. Previous PESEL UKR number (if you have a card or certificate).

Step 3: Fill in the application form
